If you’d like to establish as a new patient, please download and complete the below paperwork. After uploading it back, just give us a few days to process the paperwork and then someone will call you to schedule your first appointment. If you don’t receive a call after 5 business days, please feel free to call us and check on it.

Prior to scheduling a first appointment, all patients must complete new patient paperwork. Please don’t leave anything blank, including signatures. You’ll need to use Adobe Acrobat (not a browser) to apply digital signatures. When you’re ready, you can securely upload it back to us here. We highly recommend completing this paperwork electronically so it’s easier to read. You can also bring us your completed paperwork in-person if you prefer. Just swing by the office during our normal business hours.

Please note: Our staff members only speak English. If you cannot speak or understand English, please bring someone with you to translate. Nuestro personal solo habla inglés. Si no puede hablar o entender inglés, por favor traiga a alguien para traducir.

 

We are currently in-network with Medicare, Tricare (but not Prime), and Blue Cross Blue Shield (but not Blue Select). If you’re insured with another company (or have a Blue Select policy) or if you’re uninsured, you’ll need to self-pay for your care at the time of your appointments.

Tricare Prime Patients: We are unable to serve as your Primary Care Physician (PCP) and therefore unable to send referrals for you. You can still be seen at Access Medical Group as a self-pay patient, but referrals to other providers will need to come from your on-base PCP.
